We are using a new online service called Register with a GP surgery that makes it easy to register with this GP surgery.

Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

paper form is still available if you need one.

We do require our new patient questionnaire to be completed by all patients as it can sometimes take time for your medical records to arrive from the previous surgery.  The questionnaire will now be available when you complete your online registration, please ensure this is completed as thoroughly as possible’ to ensure your records are accurate.

All new patients are required to have a health check with the nurse, ideally within one month of applying.
